4/29/2023 EnchantaVaughn Approved 1. Behavior: Upon visiting client for in-home services, it was observed client was playing safely in her room with her little sister. Client was excited to see her counselor, as she gave her a hug and hi-five. The family communicated great dialogue and interactions as the session began. Client displayed appropriate manners and temperament toward QMHP. Client was fully engaged in family activities, as seen sharing and taking turns. Client appeared to be in a good space today. 2. Behavior: Client shares her daily experiences at home, by reflecting on whatâs it like to be at home full time. Client expressed she couldnât wait for more things to open up because she wanted to travel and go to different places during the summer. Client was thankful that her counselor provides one on one help when she needs it and expresses happy thoughts about being in session. Client demonstrates objectives, due to exhibiting anger and impulse control. 1. Intervention: QMHP explored positive self-talk, instruction modeling, and social communication skills to encourage client to speak in a confident tone and communicate her thoughts, emotions, and feelings. QMHP educated client on behavior modification to help client identify what behavior she needs to decrease and avoid daily. QMHP taught client relaxation, calming, and guided imageries, to help her reduce and regulate her overactive behaviors around her loved ones and friends. 2. Intervention: QMHP will brainstorm strategies with client and encourage self-monitoring. QMHP will provide mediation and relaxation techniques, as well as, simple praises and encouragement to continue the positive behaviors she was exhibiting. QMHP aided this intervention with client on social skills. QMHP commended client on utilizing coping strategies to increase her understanding of feelings and emotions. QMHP provided and demonstrated positive and empathetic acceptance, as talk therapy and active listening skills were applied. 1. Response: Client communicated positive affirmations and self-talk as she was greeting her counselor. Client shared some of the things she does at home to help her relax. Client has become more involved in age appropriate social activities and was given positive reinforcement for these choices. Client demonstrated expectations and responded well to feedback. Client verbalizes positive feelings about herself but recognizes areas she needs to work on. 2.
